Young Senior Female Citizen Happy Face With Nurse

Home Care

It takes a village to support and care for loved ones. Our home care service team brings our engaging care plans and programming to your door, so children and adults with different abilities can take part in the community and connection normally offered at our Community Center. Our home care services give clients and families more quality time together at home.

Home Care Basics

Our team has worked with people of different abilities for nearly 40 years. We create programming designed to enrich and engage clients mentally and/or physically. Because we know different people, families and caregivers have distinct needs, we work to deliver the right set of solutions and care plans for people with different abilities each day. Our home care services run from 8 a.m. to 5 p.m. Mondays through Fridays and include:

Nurse Assists An Elderly Woman With Skin Care And Hygiene

  • Health Services
  • Caring, Experienced Staff
  • Therapeutic Activities
  • Personal Care and Grooming
  • Social Connection and Daily Activities

Enrolling in Home Care

We’re excited to work with you and your loved ones. We have a four-step enrollment process for members so we can learn about them, their interests, health care needs and family needs.

1. Fill Out Our Online Form or Give Us a Call

The first step in the Enrollment Process is to complete the Enrollment Application. Completed forms can be returned to Alison Sarkozy, operations manager, at info@activelivingforall.org or by mail to 201 Hillside Court, East Lansing, MI 48823.

2. Interest Meeting and Tour

We’d love to meet you. Call us to arrange an in-person meeting that will include the operations manager, who is a dementia care specialist; our registered nurse; yourself; your potential member; and any other interested family members. The meeting takes about two hours, and we are happy to provide support for your loved one during the meeting.

3. Physical and TB Screening

Prior to your loved one enrolling in our home care services, a physical and TB screening must be completed by a doctor. Our operations manager will provide you the information you need to get started.

4. Choose Your Days

Now it’s time to join us! Our operations manager will work with you to schedule days and determine needs. Once you finish this step, your loved one is ready to have fun with us.
